Qualifications for Enrollment

CNA Program

Enrollment in the Nurse Aide Program (CNA)  requires an enrollment agreement and the following:

• Evidence of Education: Minimum 8th grade and above
• Government Approved photo ID
• Copy of social security card
• $280 registration fee. (Refundable)

Certified Medication Aide (CMA) Requirements

Enrollment in the Nurse Aide Program (CNA)  requires an enrollment agreement and the following:

• Enrollment agreement
• Government Approved photo ID
• Social security card
• $225 registration fee.
• CNA certification (Working in a Nursing Home or CNA applicant working in Assisted Living ) for 90 days.
